Context tracks durable fragment identities and explicit invalidation, so a changed fragment need not trigger a whole-repository reread.

Quiet state retains monotonic cursors and wake identities across restarts. A new failure, decision or terminal result can wake the agent; unchanged polls stay outside the model.
